A little prologue

A knock on Doctor Cold stone's office by Mr. Bond.

Doctor : come in please

Patient : thank you doctor, good morning sir!

Doctor : Good morning my dear, please have a seat. How are you doing

Patient: Doctor, hmm I am not fine o , or do I?

Doctor : laughs... If I may ask what is the problem?

Patient: Now you are talking doctor! See doctor, I don't usually have enough erection, and I also ejaculate in less than two minutes during sexual intercourse . Introduction

Meanwhile let's quickly talk about the male reproductive system, sexual intercourse, the physiologic mechanisms, processes and the whole lot of activities that happens during sexual intercourse taking the male as case study today. While you are enjoying that lovely moment, do you know what really happens. Do you know that it is the inflow of blood into the penis that makes it erect. Do you know the difference between spermatozoa and semen. All these and several more will be revealed in this article.

It's another great opportunity and privilege to share with us today a topic that any where it is taught, people are usually more eager to listen to. Sex is sweet and it is that period when emotions and feelings run high. It is that moment when both partners forget about beauty, the only thing that matters is the feeling of excitement in the process.

As one of my friends will most times say whenever he talks about sex

"during sex, no one thinks about the nature of the area between the two legs"

In as much as sex has its own advantages, such as boosting of the immune system, stress relief , reduction in risk of hypertension, improve the female bladder control, reduces pain and anxiety, helps you have a better sleep and many more, it could as well be the most deadly weapon if not controlled especially in the cases of multiple sex partners.

what really is sex?

If you ask me what really is sexual intercourse, and reason why people are involved in the act, well this will be my honest response

"sexual intercourse is an act that involves the process where by a male reproductive organ is thrust into a female reproductive tract. The event resulting to release of semen into the female and this consequently leading to the fusion of the sperm and egg which finally gives rise to a zygote. Sex gives that unique satisfaction that nothing else can which is obviously why many indulge in it"

I tried as much as possible to summarize a lot of things in the definition I gave above but believe you me, there is a whole lot of event that occurs during those processes. Beginning from the arousement of both the male and the female counterparts, followed by increase in the muscle tension and the sexual intercourse proper. Yes, everyone knows that sex is sweet, even those that have not indulged in the activity will still blindly and affirmatively tell you that sex is sweet but the processes involved in this act is overwhelming.

I am going to explain a whole lot of concepts in this article, I will expose those hidden mechanisms that bring about the excitement of sex so that you get to understand that there are certain things that happen inside of you of which you are not totally aware of neither are they fully within your power to control. Let's continue!

Physiological events that occur during sexual intercourse

Man as a case study

Let's begin with the sensitization stage, a stage where the man is aroused and in the mood for sexual intercourse. A lot of factors come into play after arousement. The first and most important is erection of the male genitalia - the penis. The penis must be on erection before sexual intercourse. The big question now is, how does erection occur? Have ever pondered on this before? Why is that whenever you feel sexually aroused, the Penis responds by erecting but later in the long run becomes flaccid once the feeling of arousement is gone. Here is the reason!

Mechanism of penis erection

Erection of the penis is initiated first by dilation of the the arterioles of the penis. This action allows blood to quickly and gradually rush into a tubular tissue in the Penis thereby making it strong and erect. Once this happens, the penile erectile muscle tissues surrounding the shaft of the penis - the corpora carvenosa and the corpus spongiosum compress the veins and by this way doesn't allow the outflow of blood hence ensuring the tugor of the penis.

This corpus spongiosum is that muscle particularly muscle that contracts during ejaculation and what is assist in doing is to force the semen directly into the vagina while the corpora carvenosa plays the major role of keeping the penis erect. The chief major muscle that actual propels the semen out of the urethra is the contraction of the Bulbocavernososus muscle, this muscle is very strong and you cannot in any way prevent it from carrying out its job once it has been stimulated.

When ever you hear of erectile dysfunction (inability to obtain an erection) or impotency, the first thing your mind should think of is these two muscles. They are usually innervated by the parasympathetic nervous system most importantly the fibres of pelvic splanchnic nerves (nervi erigentes) of the body and in a situation whereby thy are not stimulated, the penis will not rise. Though the two muscle are not their only ones solely associated with impotency, other factors can also contribute.

Penile erection ensures that the spermatozoa is safely transportated all the way from the site of production and storage - the testes, which is enclosed by the scrotal sac to the site of ejection into the female.

Now back to the matter, the nerve fibres I mentioned above release acetycholine a neurotransmitter and a vasodilator. The nervi erigentes contains enormous amount of Nitric oxide synthase aka NO synthase an enzyme that catalyses the formation of Nitric oxide. Hmm, nitric oxide? What has this got to do with ejaculation?๐Ÿ™„

Let me burst your bubble, without this chemical, you can never last long in bed and in fact, you cannot get an erection. So now you see that it has a significant role to play in conjuction with the two muscle I mentioned above.

How does nitric oxide keep the penis erect?

The mechanism is quite simple, upon stimulation of the penis, nitric oxide is produce from the carvenous nerves through action of the enzyme Nitric oxide synthase. After production, the nitric oxide then activate the enzyme Guanylyl cyclase which helps in the conversion of Guanosine triphosphate (GTP) to **Cyclic Guanosine monophosphate (cGMP)**through dephosphorylation (removal of phosphate group from the GTP) . This formed cGMP is a very active vasodilator of blood vessels. Its increased concentrations in the penile tissues causes the veins to dilate and in the process allow blood to flow into the penis thus making the penis erect and turgid. As long the concentration of the cGMP is high, the penis won't be flaccid. To ensure that the penis is erect, acetycholine also released and it's job is to ensure that a more sustained amount of nitric oxide is released. I think it is also necessary to state here that, the CGMP released also block or inactivate calcium receptors. If you recall, calcium plays a crucial role in the contractions of muscles so it's inactivation will obviously favour the relaxation of the muscle.

The cGMP maintains the tugor of the penis by causing the desphosphorylation of Myosin light chain through Protein kinase enzyme in the contractile system of the penis muscles thus, preventing the interaction of Myosin light chain with actin filament. If you recall from your knowledge of biochemistry of muscle metabolism, when actin - a muscle protein is blocked from interacting with Myosin light chain, relaxation of muscle occurs. In Summary, the inflow of blood into the spongy tissues of the penis is actually what keeps the Penis erect while ejaculation is due to muscles.

What then makes the Penis flaccid after ejaculation

Erection is under the parasympathetic nervous system of the body while flaccidity is under the sympathetic nervous system. What this means is that, during flaccidity the sympathetic part of the autonomic nervous system takes over. we have been able to establish for a fact that nitric oxide plays a major role in maintaining the tugor of the penis. Once a man reaches the climax of the sexual intercourse by releasing semen in the process of ejaculation, the whole mechanisms that worked together to keep the penis erect automatically reverts and the penis returns to its flaccid state. Let's quickly study how this happens.

The main thing that happens is simply the reversal of all the above processes but these is catabolysed by different enzymes. The action of Nitric oxide synthase which produces nitric oxide is first inhibited so that subsequent enzymes can as well be blocked. The muscles compressing the arterioles are released and this allowing the outflow of blood from the pennile spongy tissue. Immediately the blood flows out, the erected penis falls gradually falls and return to its normal flaccid state.

The major enzymes implicated in the flaccididty of the penis after erection are the phosphodiesterase enzymes and what they do is to inactivate the cyclic nucleotides thus, inhibiting their erectogenic functions. Once this enzyme exerts its inhibitory action, the whole process of erection is automatically halted.

Men who don't last long on bed usually experience quicker inactivation of the enzymes involved in the maintaining the tugor or rigidity of the penis. They attain climax or orgasm very fast. In order to ensure that the penis remains erect during sexual intercourse even after attaining climax, drug called sildenafil aka viagrais usually administered.

How does sildenafil citrate (Viagra) works?

Most porn stars utilize this drugs a lot and it is also used to treat impotency. The normal time it should take a man to ejaculate is roughly 8-10minutes of continous stimulation of the penis whilst it is thrust in and out of the vagina. Using this drug treats impotency and make men last longer on bed by prolonging the action of nitric oxide which has a relaxant effect in the smooth muscles of the corpus carvenosa, prolonging increased blood flow into penis which results to maintained erection.

This drug ensures that the nitric oxide synthase activity is never inhibited. Sildenafil prolongs the time it will take for a man to reach climax or orgasm ,obviously the sensitivity of the shaft of the penis will be reduced as well, even the nerves will be made less sensitive.

Now the penis has been erected, what follows?

The penis is now erected waiting to release semen. Without erection, it will be quite impossible to release semen from the genital organ. Let's briefly look at the whole physiologic activities that occurs before semen release.

Ejaculation is simply the release of sperm as well as seminal plasma. Ejaculation process basically has two main stages, the stage one is called the emission stage and the second is the ejaculation proper. The emission stage is the stage where several glands that produce fluids that reduces the acidity of sperm cells and also enriches the sperm cells with several nutrients they need for migration are fully involved.

During emission, the sperms cells are moved from their site of production- testes which is housed in a safe structure called the scrotum that distends from the body. Are you still pondering about the reason why the scrotum is distended from the body and also why during changes in temperature, the scrotum moves farther from the body or closer as the case may be? Here is the reason!

"Sperm production occurs at a temperature lower than the body's temperature. The temperature of the body which is about 37 degree Celsius is not quite suitable for the formation of sperm cell, hence the body takes care of this problem through distension of the testes away from the body and also the relative movement of the scrotum during excessive hotness or coldness of the body"

Little experiment

If you think this is not so real, here is a little experiment for you to try out. Kindly go inside your closet with a container of cold water and pull off your trouser and undies to expose your God given two lovely balls below your penis. Now, gentle pour the cold water on the scrotum, carefully observe the scrotum gradually constrict and retract towards the body.

Haha ๐Ÿ˜€, yeah for real, it will retract. Allow for about two hours for the temperature to return to normal, go back to the closet with another container of warm water, repeat same thing and observe the scrotum distend further from the body. You can as well drink hot tea to raise the body temperature, definitely the scrotum will distend. That is to tell you that the scrotum is temperature sensitive.

Back to ejaculation process! like I earlier stated, the sperm cells are moved from the testes and in particular from the epidydimis, through a very strong contraction, it then travels through the the vas deferens into the ejaculatory duct located around the prostate gland. The sperm receives a lot of secretions from the ejaculatory duct, the prostate gland, the seminal vessicle , cowpers gland and the bulbourethral gland. These secretions reduces the acidity of the the sperm so as to enable it's potency and motility.

The second stage is the ejaculation proper. Now that the sperm has received fluids it needs, it is no longer regarded as sperm, rather it is called a semen. Before ejaculation proper ocurrs, the the urethra has to be flushed off with special mucous-like secretions from the bulbourethral gland and the urethral gland. The essence of this is to remove or neutralize any trace of urine and it's acidity that may may affect the viability of the semen to be released.

Once this happens, the semen are released through a very strong spasmodic contraction of the bulbocarvenosus muscle located below the penis and this muscle covers the corpus spongiosum. The strong contraction helps to forcefully push and eject the semen out of the penis. It is very important to as this point to state that once ejaculation has started, it is no longer under your control, it automatically becomes a reflex action which you cannot in anyway stop. Yep! That's all about the whole process leading to release of semen through the penis. Hope you enjoyed reading this.
